Of course, there's no reason why you can't simply put the 
documentation on the Wiki, and people can add comments to the
bottom anyway.  And periodically, comments can be refactored
directly into the actual text.

But, for more fine-grained commentary, annotation tools such as
CritSuite [1] or Annotea [2] might be appropriate.  CritSuite,
though it's pretty old, seemed to work pretty well and not require
any client support.  You just need to set up a crit server, and
then you can hit URLs like:

	http://crit.org/http://www.planet-lab.org/

That'll automatically enable annotation viewing and then you could
just click through and use HTML forms to make additional
annotations.  (Unfortunately, crit.org appears to have gone down
for lack of funding.)

Annotea might appeal to the XML fans but client support seems to
be lacking.
